Scott & White Health Plan is offering Supplemental Cash Income Policies to the employees of Scott & White. You will have the option of three different income policies:

To apply for these policies, carefully review the Outline of Coverage for each policy for which you are applying. Download and complete the two-page combined application and forward to:
