
PLAN CHANGES YOU'LL WANT TO KNOW ABOUT!
That's right. As a SWHP member, you are not required to designate a PCP. We do encourage you to select a physician who will oversee your care, but this is no longer a requirement. If you do elect to designate a PCP, please do so from the following providers:
Family Practice physicians treat all age groups from newborns to the elderly. They provide routine medical care, referrals to specialists and some minor surgical procedures.
Internal Medicine physicians treat patients 16 years of age and older and provide routine care as well as referrals to specialists.
Pediatric physicians treat children up to age 18 and provide routine care as well as referrals to specialists.
If you choose to designate a PCP, consider which clinic location would be most convenient for you. Each person enrolled in your plan may select his or her own PCP. Please refer to our online provider directory.
Another big change to your plan is that SWHP does not require a referral from a primary care physician before you can access a specialist. Just simply call the specialist's office and make an appointment.
Due to the nature of some specialties, some physician offices may require a referral from a PCP prior to making your appointment. This is the choice of that physician's office and not a requirement of SWHP.
